Yeah, after reviewing this build once more, the differences at the top of my head is Im not using a cosmic siege or Infernal. Opted for octopus medal and Headband to spread it out more. I stopped using m'kraan and already had a rank 5 Gungnir so I have it on while leveling warlock eye.
I think I have like 80 something INT, too lol. The side goal on this was easy 4℅ adding up, plus crit dmg, and 80 = shave 2s off medkit with the option to shave a third second off using cosmic ring. Haven't decided on Tome of oshtur for fun.

running both this build with my rolls and my 'f**** DEF' version, i guess about 15-20s slower than you, depends. didnt time myself, just used my sig cooldown timer and made the best guess, added like 3-5s to the time since i dont use sig right away.
my rotation is slightly different than yours. i saw your time gem page with 200 points for atk spd and movespeed. i dont use teleport, opted for more movement speed myself.
i use crumble fools without a cooldown. get it up to around 500k brutal dmg or so and by the time you use a handful before the next worthless cretin is up, same dmg?
also crumble fools has a better recovery time to use a power after it with relative ease on the animation transition from one to the next. whereas, i dont like using torment after doom burst. while your ttk doesnt lie and it looks to be good enough rotation for best times, i gotta go with playability on my part.
also used the last talent in tier 1 for the 10% dmg increase, w/ eldritch, 20%. chose foot dive over worthless cretin unless boss terminal farming.
and you tested ultron encephalo beam and iron legion??? isnt better than crown?? because in legion you have +1 stats, +200 brutal and +300 summon power to will of doom.
To answer your first question, our enchant options for slots 2, 3 & 4 are limited to essentially utility stats like - Defense, Dodge, health, Spirit etc... so i went with spirit to help increase Doom's natural regeneration rate.

Nevermind that, was wondering if you tested Helm of Doom vs Jinn vs Cobra's hood. And if so, what are the differences between them?
My Helm of Doom is better rolled than cobra so the hood loses. Not only that, the hood is a pain in the ass to reroll. The health on the Helm is the only red ℅. Jinn is easily the winner. Was curious about those 3 slot 5 uniques. I use jinn on everyone lol
Yeah i should have the build updated today sometime with all the new infinity picks/updated hero synergies, Not much has changed gear-wise (even tried out a Cosmic slot 2) but his own slot 2 was still performing better in tests.

preferably imho you would want Nuetral damage types (since Cretin/air defense are physical) but since the larger portion of Doom's damage is mental in this setup then yes a Medusa insignia can still work out.
